Application Checklist

The application must be submitted electronically to BWF in its entirety by the due date.  The following checklist is provided to assist you in compiling and completing all the necessary application materials.

Sections of the application:

  • General proposal information.  Completion (data entry) of the general proposal information into BWF's on-line grant submission system.
  • Project Plan and Attachments.  The Project Plan and other attachments uploaded as PDF files. 

Project Plan.  The Project Plan consists of the following seven components:

  • Executive Summary (one page)
  • Project narrative (five pages)
  • Recruitment plan (one page)
  • Evaluation plan (one page)
  • Staff capacity (one page)
  • Organizational capacity (one page)
  • Replication and dissemination (one page)

Other Attachments:

  • Curriculum vitae of the program director and two project staff (no more than three CV's; no more than two pages per CV, uploaded as CV1, CV2, and CV3)
  • Budget form
  • Budget justification
  • Letters from Project Partners (optional, up to three letters)
  • Documentation on Nonprofit status
