Progress and Financial Reporting


Princeton University 2009 Timeline 

April 1 - May 12:  BWF Trainees complete on-line Trainee Report and Survey  
May 13 - May 29: Program Coordinator finalizes on-line trainee data 
June 1:  Deadline for hard copy submission of progress and financial reports


Please format your report as follows:

  • Submit the completed Program Summary Cover Sheet as a cover sheet to your Progress Report.
  • Submit five (5) single-spaced and double-sided copies on 8 ½ x 11-inch,  three-hole-punched paper, using 12-point font size.
  • Number each subsection separately.  Collate all sections so that the original and each of the five (5) copies contain all necessary materials.
  • Fasten each report with one binder clip or one rubber band.

Do not use:

  • staples
  • paper clips
  • folders
  • colored paper
  • non-BWF cover sheets
  • dividers or tabs
  • Submit the report to

Program Summary Cover Sheet


The Program Summary Cover Sheet (see form below) must be completed online. To fill out the form, click on the blank line and begin typing.  To advance to the next line, press the keyboard "Tab" key until the form is completely filled out.  Be sure to press the return key after the last entry to complete the process.  When you are finished, print the file and include it as the cover sheet for your report. 

The Program Summary Cover Sheet for your progress report should contain the following information:

1.  Institution
2.  Program name
3.  Program web site
4.  BWF Request ID number for your grant
5.  Progress report period
6.  Program directors' names
7.  BWF trainee information (at each training level)

  • Number of BWF "Fellows" (those receiving BWF stipend support)
  • Total number of BWF "Associates" (no stipend support, but actively involved in program activities)
  • Number of applications received for BWF fellowships
  • Number of new BWF "Fellows" appointed during reporting year
  • Total number of former "Fellows" (program alumni) no longer supported
  • Total number of former "Associates" (no longer involved)

8. Date report submitted  


Introduction:

Each progress report will be read and evaluated by members of BWF's Scientific Advisory Committee. Evaluation criteria include:

  • Scientific rigor of training provided
  • Addressing interdisciplinary "language/culture barrier"
  • Trainee qualifications/selection
  • Trainee accomplishments
  • Cross-departmental faculty involvement
  • Creating community for trainees
  • Use of BWF funds (primarily should be used for trainees)
  • Institutional impact
  • Contribution to BWF program goals

Please prepare the report with these criteria in mind.


Part 1:  Program Report (to be completed by Program Directors)
 

A.  Briefly (maximum 3 pages) review the original plan and describe program accomplishments and all training activities supported by the BWF award during the reporting period.  Include descriptions of the following where applicable:

  • how trainees were selected
  • program activities
  • any curriculum developments or changes
  • any changes in administration of the program
  • plans for the coming year
  • any other sources of support for this program
  • dissemination on the internet of course material or
  • workshop proceedings, with links.


B.  Provide a brief description and rationale for any funds disbursed to individuals other than trainees, showing titles of projects, names, titles, and departmental affiliations of recipients, and references of any resulting publications.

C.  Provide a chart listing names of all BWF trainees at each training level during the reporting period, indicating the inclusive dates of their fellowships. Separate according to training level. Within each training level, indicate whether trainees are "Fellows" (receiving stipend support from BWF) or "Associates" (not receiving stipend support, but may be receiving other or no BWF support). The number of names in each category should match the numbers entered on the Program Summary Cover Page. Each trainee listed should complete the trainee report and survey (Part 3).

Part 2: Program Evaluation (to be completed by Program Directors)
 

A. Briefly (1 page or less) describe specific efforts and plans to evaluate the success of the program, including what kind of data has been collected, and names of any internal or external program reviewers. What cluster of outcomes should be used to measure the success of your program? To what should your program be compared?

B. Briefly summarize how your program is doing according to the measures identified in (A) above.

C. Summarize any institutional changes made as a result of the award, as well as the impact of the program on trainees not receiving BWF stipend support.

D. Provide a chart listing names & departments of any new faculty collaborations that have arisen as a result of the program, along with project title, publications, and funding source if any.

E. For all trainees who have completed or left the program to date, list current positions and institutional affiliations, and contact information where known. Please indicate the inclusive dates of their involvement in the program, and their training level. Where possible, describe the extent of their current involvement in research, including research grant support and representative recent publications.

Part 3:  Trainee Report


An on-line report for each BWF trainee must be completed, printed, and included as Part 3 of the Progress Report. The Trainee Report form will be available as part of the BWF Interfaces On-line Reporting System (IORS) for a limited time.

Tainees have access to complete their on-line Trainee Report and Confidential Survey. Trainees will not be able to enter/revise data after their program deadline; access will be “view only.”

May 13, 2009 to Friday, May 29, 2009 by 3:00 p.m. Eastern Time

Program Coordinators have access to finalize Trainee Report data. Program Coordinators will not be able to enter/revise data after their program deadline; access will be “view only.”

Program Coordinator Instructions for completing the Trainee Reports for your program are provided. Program Coordinators are responsible for verifying the accuracy of information included in the Trainee Reports for their programs.

Part 4:  Financial Report (see form below)

The institutional financial officer should complete the annual report on award expenditures. Provide five (5) copies on three-hole-punched paper, collated as Part 4 of the progress report. The Financial Report may be submitted under separate cover.

To complete the on line forms, click on the first blank line and begin typing. To advance to the next line, press the keyboard "Tab" key until the form is completely filled out. Be sure to press the return key after the last entry to complete the process.   

  • If you submitted a Financial Report last year, information provided in that report should be used to calculate the Cumulative to Date figures. 
  • The subtotal fields in both columns will automatically calculate as you enter the relevant numerical data.
  • The "Balance" in both the Fiscal Year and Cumulative to Date columns should be equal.
