Forms and Resources

Templates for the application process are provided below.

Both MS Word and PDF versions of the forms are available.  If you choose to use the MS Word version, you must correct any changes to the form resulting from input of data prior to converting it to PDF format. Data should be completed in the space provided. The form must respect the original margins on all sides.

If you choose to use the PDF version, you must use Adobe Acrobat to complete and save the form to your system. (Using Adobe Reader will not work properly, neither will using Preview on a Mac.)

If you do not have access to Adobe software on your system, a list of PDF generator software is available on the grants.gov website at: http://www07.grants.gov/help/download_software.jsp#pdf_conversion_programs. Please note that BWF does not endorse or take responsibility for any of the software programs listed on the grants.gov website. There may be other scanning equipment options available to you.

The Items below must be combined into one continuous PDF file in the order shown.  You will then upload the PDF file as an attachment to your application in BWF's on-line application submission system.

  3. Biographical Sketch of Principal Investigator and all Collaborators/Partners (NIH-style) 
  4. Letters of Support from all Collaborators/Partners (10 maximum)
  5. Scientific Abstract (one page; template required)   
  6. Research Plan (12-page limit)
  7. Budget (template required)
  8. IRB Certification
  9. Animal Care Certification
  10. Reprints/Publications (10 maximum)
