The entire application must be submitted electronically through BWF’s grant application system by October 3, 2011, 4:00 p.m., Eastern Time.  The following checklist is provided to assist in compiling and completing all necessary application materials.

Did you remember to complete or obtain the following attachments and upload them as one (1) PDF file in the following order?

1. Scientific Abstract (one-page limit; template required)
2. Research Plan (required; six-page limit)
3. Publications (up to three; one required and two optional)
4. Faculty Sponsor Letter of Nomination (required)
5. Chief of Service Letter (if applicable)
6. Ph.D. Advisor Letter (if applicable)
7. Candidate's Biosketch (four-page limit; template required)
8. Bibliography (optional)
9. Personal Statement (template required)
10. Signature Page Form (template required; three-pages including the page with signatures and two-pages with contact information for the application supporters)

Did you request two confidential/blind Letters of Recommendation?  Have these letters been uploaded by the recommenders?
