Application Checklist

The entire application must be submitted electronically through BWF’s grant application system by October 1, 2012, 4:00 p.m., Eastern Time.  The following checklist is provided to assist in compiling and completing all necessary application materials.

  • Nominating Institution Information
  • Proposal Information 
    (*Did you remember to include your name in the "Project Title" field in the format of "Last Name, First Name - Project Title"?)
  • Lay Abstract
     

Did you remember to complete or obtain the following attachments and upload them as one (1) PDF file in the following order?

  • Cover Page (template provided; required)
  • Scientific Abstract (one-page limit; template provided; required)
  • Candidate's Biosketch (four-page limit; required)
  • Research Plan (six-page limit; required)
  • Submitted Manuscripts
  • Bibliography (optional)
  • Personal Statement (template provided; required)
  • Faculty Sponsor Letter of Nomination (required)
  • Chief of Service Letter (if applicable)
  • Ph.D. Advisor Letter (if applicable)
  • Signature Page Form (template provided; three-pages including one page with signatures and two-pages with contact information for the application supporters)

    Did you request two confidential/blind Letters of Recommendation?

    Have these letters been uploaded by the recommenders?

    Note:

    • BWF does not require a budget at the time of application for the CAMS program.
    • Letters of collaboration are not necessary and will not be accepted.
